We see this confusion in almost every first-time injectable consultation. The two most well-known treatments are doing very different jobs — and treating one as if it's the other tends to make people unhappy with results.

Anti-wrinkle treatment
Targets movement-related lines — the lines that appear when you frown, raise your eyebrows, or smile. Works by relaxing specific muscles so they contract less strongly, allowing the skin above to crease less. Done across the upper face: forehead, between brows, around eyes. Lasts around 3–4 months.
It does not add volume. It does not change facial structure. It does not treat the lips themselves.
Lip filler
Targets volume and shape in the lips specifically. Hyaluronic acid placed at specific points on the lips to add subtle volume, refine shape, or correct asymmetry. Lasts around 6–9 months.
It does not relax muscles. It does not soften lines on the forehead, between brows, or around the eyes. It is not a treatment for ageing in general.

Anti-wrinkle is for lines from movement
Forehead lines from raising eyebrows. The 11s between brows when you concentrate. Crow's feet when you smile. These are dynamic lines — they appear with muscle action — and anti-wrinkle relaxes the muscle to soften them.

Lip filler is for volume and shape, not lines
Subtle lip enhancement, asymmetry correction, definition of the Cupid's bow. Filler adds volume; it doesn't relax muscles, doesn't affect movement, and doesn't treat lines elsewhere on the face.

Vertical lines on the upper lip can sometimes soften with very small anti-wrinkle doses, sometimes need skin boosters or chemical peels for the surface texture. The honest answer is rarely “just use one or the other” — we'll tell you which (if any) suits.

When the Answer is Neither
When the right answer is “not yet”
A meaningful proportion of first-time consultations end with us recommending neither treatment — at least, not today. That's usually for one of these reasons:
- The thing that's bothering you is volume loss in the cheeks, jaw, or temples — not lips and not lines. Different conversation.
- The lines are static (visible at rest) rather than dynamic. Anti-wrinkle softens dynamic lines well; static lines may need combination treatment with skin boosters or microneedling.
- Your lips are already at a volume where adding more would start to distort the shape.
- Your skin barrier is currently irritated or compromised — we'd typically pause for skincare focus before any injectable.
- You're pregnant, breastfeeding, or have a medical reason we'd need to discuss.

Anti-wrinkle treatment uses a purified protein to temporarily relax specific facial muscles — softening lines that come from movement. Lip filler uses hyaluronic acid (a sugar-based substance the skin makes naturally) to add subtle volume or shape to the lips. They're different products, in different places, doing different things.
It depends on the kind of lines. Vertical “smoker's lines” or fine lines from lip movement may soften with very small doses of anti-wrinkle, sometimes combined with hydrating treatments. Volume-related changes around the mouth (corners turning down, upper lip flattening) tend to be a filler conversation. We'll look at the area together at consultation and explain which is appropriate.
Sometimes — but for first-timers, we usually recommend doing one at a time so you know how each treatment affects you. Combining at the first visit makes it harder to know what's working, what's settling, and what (if anything) needs adjusting.
Both involve injections; both use a topical anaesthetic where appropriate. Lip filler is the more sensitive area but the filler itself contains a numbing agent. Anti-wrinkle injections are typically described as a brief sting at the injection point. Most clients describe both as tolerable rather than painful.
Lip filler typically lasts 6 to 9 months. Anti-wrinkle treatment lasts around 3 to 4 months. Both are absorbed by the body gradually; you can choose to repeat or stop at any point.
That's the consultation's job. Many first-timers come in with one in mind and leave thinking about the other (or neither). We'll look at the specific areas you're interested in, explain what each tool can and can't do, and tell you honestly if the right answer is to wait.
